This job serves as a key member of the health care team. Assumes responsibility for the management of assigned coaching cases to ensure patients have ability to improve for the duration of their participation in the Wellness program. Using behavior change theory and motivational interviewing techniques, works together with the patient to identify and monitor progress on meaningful goals and lifestyle changes that will help patients develop healthier habits, leading to better health outcomes. Assists patients with program adherence by monitoring adherence to therapy and wellness appointments, testing and outcome collection, home exercise programs and other lifestyle factors. Responsible for identifying and providing information and education to patients about available health interventions to increase patient engagement, capability, and accountability. Coordinates with therapists, advance practice providers, nurses, physicians and other care providers, maintains documentation as required by law, and actively participates in special projects, as needed.Location: OLG Orthopedic Hospital
Full Time
Education
Required - Bachelor’s degree in health, human services, pastoral care, education or related field
Preferred - Master’s degree in health, human services, pastoral care or educational field
Work Experience
Required – 5 years’ relevant experience
Preferred- Experience in health care, human services, customer service, education, fitness or marketing fields
Certifications
Required – Basic Life Support (BLS) Certification from the American Heart Association (AHA)
Preferred - Health Coaching Certification by WellCoaches®, or actively pursuing
Knowledge Skills and Abilities (KSAs)
- Must have computer skills and dexterity required for data entry and retrieval of information.
- Effective verbal and written communication skills and the ability to present information clearly and professionally.
- Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
- Excellent organizational skills.
- Ability to demonstrate strong communication and interpersonal skills.
- Team player
- Demonstrates good judgement and conflict resolution skills
- Excellent interpersonal skills
- Self-directed with ability to work independently under minimal supervision
- Excellent skill at building rapport with patients and care teams
- Must be able to work in a fast pace environment with the ability to juggle and prioritize competing tasks and demand
- Ability to fulfill program needs at other sites, as needed.
Job Duties
- Introduces Wellness program to patients/clients and schedules regular touchpoints, develops effective relationships to increase their motivation, capability, engagement and programmatic participation.
- Provides patient/client education, information and encouragement regarding available health interventions including, diet, fitness, and goal setting to improve health outcomes based on individualized needs.
- Manages records and provides appropriate documentation to track patient’s participation and outcomes. Ensures all outcome data is collected throughout and at end of program. Assists with report generation at program level.
- Effectively works with Therapy Team, including providers to optimize participant outcomes.
- Assists with interviewing potential candidates and training new hires.
- Assists with additional departmental projects including development of presentations and education material.
- Demonstrates continued dedication to the field of health coaching through continuous education units (CEU) and pursuit of new certifications.
- Represents the department events and community events.
- Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
- Other related duties as required.

Very Heavy Work - Exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Heavy Work. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
Normal routine involves no exposure to blood, body fluid or tissue and as part of their employment, incumbents are not called upon to perform or assist in emergency care or first aid.
The incumbent has no occupational risk for exposure to communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
