This job assesses and manages the bio-psychosocial needs for a patient in the ambulatory setting and applies the nursing process to provide quality patient care.Education
Required - Registered nurse diploma.
Preferred - Associate's degree in nursing.
Work Experience
Required - 3 years of clinical experience in the delivery of patient care as a registered nurse in a diverse medical background.
Certifications
Required - Current RN license in state of practice.
Knowledge Skills and Abilities (KSAs)
Proficiency in using computers, software, and web-based applications.
Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the required job processes.
Good organizational, decision-making, and delegating skills.
High level of interpersonal and public relation skills.
Leadership skills.
Ability to handle high pressure situations.
Ability to be highly motivated, eager to learn new skills, and find creative solutions to challenges.
Job Duties
Oversees the disease registry database.
Conducts pre-visit chart review of patients.
Works with patients and families on self-management support.
Coordinates care across the care continuum (case management).
Assists in developing tools and quality improvement activities.
Performs other related duties as required.

Physical and Environmental Demands

The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
